Hazel Health

Jeffco Public Schools partners with Hazel Health, Inc. to provide free, additional physical and mental health care services for students. This a telehealth extension of support to Jeffco students enrolled at district-managed schools. These on-demand integrated healthcare services are evidence-based, culturally responsive and can be accessed at home with a parent or legal guardian.

Steps To Access Care

If a student needs immediate medical attention CALL 911. Hazel does replace the 9-1-1 system.

Eligibility

Jeffco students from preschool to 12th grade who are enrolled at district-managed schools can use Hazel with the family’s consent.

Charter school families should contact your school leadership to see if your charter school has contracted with Haze for services. 

The service is funded by ESSER and free for users. The Hazel consent form will ask for insurance information. Any student, regardless of insurance status, can use Hazel. Providing insurance information helps Hazel to better coordinate care for your child, in ways such as referrals and prescriptions.
