Hazel Telehealth Services for Jeffco Families

Jeffco Public Schools has partnered with Hazel Health, Inc. to provide a telehealth extension of mental and physical health supports to Jeffco students enrolled at district-managed schools through evidence-based, culturally responsive, on-demand integrated healthcare services, accessed at home with a parent/guardian. 

Why Would a School District Use Telehealth Services?

Our student’s physical and mental health needs must be met in order for them to learn in the classroom. Across the United States, nurses and mental health providers are in demand and there are not enough trained professionals to meet the needs across our communities.

Through the use of telehealth and digital health tools, families now have access to additional healthcare providers offering physical and mental services in support of student success.

We believe that every child should have access to the care they need and Jeffco’s partnership with Hazel Health will serve as an additional option of support to families who choose to access services

Ensuring Student Data Privacy

Jeffco takes our students’ digital privacy very seriously and understands that health-related information is incredibly sensitive. Student data privacy was a cornerstone consideration while searching for a telehealth partner, negotiating the contract and implementing the system.

Hazel Health was chosen through a competitive RFP process which included a technical review of their information security and privacy practices. Once chosen, Jeffco applied specific privacy-focused content to our contract with Hazel, holding them accountable for maintaining security and privacy practices required by the district, along with dictating data lifecycle and incident response terms.

Jeffco’s implementation of Hazel was focused on providing the minimal amount of student data necessary for operating the system. The district implemented an opt-out process which is unique to Jeffco’s implementation of the system and ensures that no student or family information is rostered into Hazel platform for families choosing to opt-out of the service.

Only the minimum amount of data necessary to associate a student with their family is entered into the system for families that have not opted-out. This basic information is necessary to create an eligible student roster, allowing for parents to access services for their students. Health information is entered into a system only after a referral is made and that information is governed by the industry standard healthcare framework, HIPAA.


Services

All Hazel Health services are available only at home with a legal parent/guardian.

  • Unlimited physical health services (including assessment and treatment) that can be accessed year-round. Services are available on-demand (no need to schedule an appointment) from 7 a.m.–5 p.m. local time.
  • Access to scheduled licensed mental health counselors for delivery of the comprehensive HEART Protocol for students via phone and/or computer at home. Referrals can be made by school staff or parents/caregivers (see below).
  • Access to follow-up and case management support for participating students, including appointment scheduling for in-person services, assistance navigating the healthcare system/Medicaid, coordination for ongoing counseling or psychiatric supports, and coordination with existing schools supports.
  • Services are accessed via multiple entry-points into care, including (but not limited to):
    • On-demand physical health visits initiated at home by a legal parent/guardian.
    • Mental health referrals from school staff, most often the school Social Emotional Learning Specialists, Counselors, School Psychologists or School Social Workers that families can activate from home, together with their student. 
    • Request for mental health services from a legal parent/guardian.
    • Self-consent by students 18 or older, with school staff completing the referral.

Steps to Access Care

Accessing Physical Health Services

  1. Legal parent/guardians can sign up by completing the consent form.
  2. Create an account.
  3. Request an at-home appointment. In your account, click Contact Doctor and send a message to request an at-home appointment.
  4. Receive the visit confirmation from Hazel Health. The Hazel Health team will schedule a visit and will contact you to confirm the time.
  5. Join the visit. At the visit time, join the visit through your account or by clicking the link in the text message you receive from Hazel Health.

Accessing Mental Health Services

Parent Referral

  1. Legal parent/guardians can sign up by completing the consent form.
  2. Create an account.
  3. To request a therapy appointment, call 1-800-76-HAZEL (42935). A Family Resource Manager from Hazel will work with you to schedule an initial visit. 

School Mental Health Staff Referral
Jeffco mental health professionals have access to refer your student. After the referral is submitted, a Family Resource Manager from Hazel Health will reach out to the legal parent/guardian to obtain consent and work to schedule an initial visit.

Hazel Health is not a replacement for the emergency room.

If a child needs immediate medical attention PLEASE CALL 911. Hazel is not designed to replace the 9-1-1 system.


Eligibility

Jeffco students who are enrolled at district-managed schools (Preschool–12th grade, up to age 21) are eligible to use Hazel Health with legal parent/guardian consent.

Charter school families: Please contact your school leadership to see if your charter school has contracted with Hazel Heath for services. 

Services are available at no cost

The service is fully funded by ESSER with no cost to the end user. The Hazel Health consent form will ask for insurance information. Any student, regardless of insurance status can use Hazel Health. Providing insurance information helps Hazel to better coordinate care for your child, in ways such as referrals and prescriptions.
