To the Parents or Guardians,

 

Your son/daughter has indicated that he/she is interested in learning more about the School to Work Alliance Program (SWAP) and the Division of Vocational Rehabilitation (DVR) which is a program that assists young adults with barriers to employment, with an IEP or 504 plan, in entering competitive, entry-level to career-path jobs. SWAP and DVR can offer various services to the young adults that will assist them in getting entry-level jobs of their interest, and provide them with the support and problem-solving skills necessary to ensure success once they are placed in a job.

 

There is an application process that is needed to make your student eligible for services with DVR, to begin the process school records such as the current IEP need to be shared with DVR,

 

  1. A parent/guardian signature on a DVR’s “Release of Information” form. This form needs to be signed, dated and returned. This is the permission to share the school records with DVR.

 

  1. A copy of your youth’s Colorado State ID, Driver’s Permit, Driver’s License. This is to establish Colorado Residency, since this is a state program, all clients must be a Colorado Resident, to obtain information to get a Colorado State ID, https://www.colorado.gov/pacific/dmv often found under Licenses. 

 

  1. Your youth’s valid Social Security number. This is needed to determine Citizenship, since DVR gets funding from the Federal Government this in needed to verify citizenship.  (This number will not be shared with anyone; it is used for eligibility and is never needed again for this purpose. The student will, however, need this number to apply for most jobs.)
